Requests for Additional Information Forms

When processing your financial aid for the 2008-2009 academic year, we may request verification of information that you reported on your 2008-2009 Free Application for Federal Student Aid (FAFSA). The following list of forms provide instructions on how to furnish our office with requested information.

 

  • 2008-2009 Asset Worksheet for Parent(s) - Use this form to furnish parent asset information that reflects asset amounts as they existed on the date the FAFSA was originally completed . Do not make changes that reflect asset amounts that have been reduced/spent/increased since the date of filing the FAFSA. The only allowable change is to correct inaccurately reported assets as of the date the FAFSA was originally completed.

 

  • 2008-2009 Asset Worksheet for Student (and Spouse) - Use this form to furnish student asset information that reflects asset amounts as they existed on the date the FAFSA was originally completed . Do not make changes that reflect asset amounts that have been reduced/spent/increased since the date of filing the FAFSA. The only allowable change is to correct inaccurately reported assets as of the date the FAFSA was originally completed.

 

 

 

 

  • 2008-2009 Household Worksheet for Dependent Students - Use this form to verify the number of people the parent(s) will be supporting during the current academic year. Also, this form is used to indicate which person(s) in the household will be attending college, the name of the institution(s) and the type of degree.

 

  • 2008-2009 Household Worksheet for Independent Students - Use this form to verify the number of people the student will be supporting during the current academic year. Also, this form is used to indicate which person(s) in the household will be attending college, the name of the institution(s) and the type of degree.

 

 

 

 

 

 

 

 

 

 

 

  • 2008-2009 Untaxed Income Worksheet for Parent(s) - Use this form to verify the amount listed by the parent on Worksheets A and B of the FAFSA or to document support received outside of what was reported on the students 1040 form, if filed.

 

 

  • 2008-2009 Veterans Benefits Verification Form - Use this form to notify OSFA of the amount of Veterans Benefits that the student will receive. Used in conjunction with the Registrar verification of enrollment in the Veterans Administration.
